We welcome new patients to both our surgeries in Wellesbourne and Kineton.
If you are new to the area, and would like to register with us, please attend the surgery during opening hours and our receptionists will give you the appropriate paperwork to complete in order for us to register you and request your medical records from your previous GP.
Registration documents should be completed and returned to our reception team with a form of identification ideally either a driving licence, passport or medical card (Name and Date of birth). If NHS numbers can be provided this is an aid to tracking the notes and speeds up the registration process.
Until we receive your medical records, it is helpful if you can complete our New Patient Questionnaire, so that we have details of any repeat medication that you are taking and any major illnesses or operations that you have had in the past.

Registering newborn babies
A NHS number is required you will receive this when your baby is registered. If older children do not have a passport then a medical card or NHS number must be provided, this can be obtained from your previous surgery.
First TIme Registering within the UK
If this is the first time you are registering within the UK you will need to provide photographic identification before we can register you.
