Change of Personal Details
Please provide a mobile telephone number wherever possible. We will use this for several important purposes including:
- Contacting you by telephone about your health if we are unable to contact you at home.
- Texting you reminders about any appointments you have booked.
- Responding by text/phone to queries you have sent us, or advising you when prescriptions/paperwork .are ready for collection.
- Advising you if appointments have been cancelled.
If you have an account on NHS app, please login here to request the change of your personal details. If you don’t have access to the NHS app, please download it using the links below. Registration will be processed online, without the need to visit the practice.
