Maternity Locum

We are looking for Locum cover for up to 5 sessions per week to cover maternity leave for 12 months, commencing end of July 2023. Ideally looking for Mondays, Wednesdays and Thursday mornings.

You will be working with a highly motivated and enthusiastic Practice Team. We benefit from being a large Practice with a team of 5 GP Partners and 5 Salaried GPs. Our Clinical Team consists of Nurse Prescribers, Chronic Disease Nurses, Pharmacist and Pharmacy Technician, Health and Wellbeing coach, Social Prescriber, a Mind Counsellor, First Contact Physiotherapist, Community Nurse for the elderly and a Frailty Nurse. The Practice has a highly organized and efficient Administrative Team to support Clinic ians. Consultations are a mix of face - to - face and by telephone.

We currently use EMIS clinical software.

We are a Training Practice for GP Trainees and Medical Students.

We operate from modern purpose - built premises in the centre of Wellesbourne village near Warwick. There is easy parking at the rear of the medical centre.

Practice Coordinator - South Warwickshire GP Federation

Job description

We are looking for an enthusiastic administrator who has excellent organisational, multidisciplinary and communication skills.

Job responsibilities

The candidate key responsibilities: (this list is not exhaustive)

  • Answering Surgery phone lines
  • Coordinating and adding clinical sessions to EMIS
  • Able to build relationships with ARRS staff, service users and allied professionals in member practices; to help identify patients who could be referred to other health care professionals within the PCN to help improve PCN efficiencies’.
  • Triage requests and queries from phone calls to the team.
  • Manage care home inbox and coordinate workflow within the team
  • Coordinating the receipt of shared care agreements or SIDC's from secondary care
  • Coordinating new patients EMIS online proxy forms and registrations for care homes.
  • Sending out hub communications to homes, pharmacies and surgeries (bank holidays, PLT, news blast for urgent matters).
  • Collation and management of shared documents on the appropriate platform ensuring the latest versions are available and are made available in a timely manner.
  • Creation of EMIS patient searches to support the coordination of project work across the PCN
  • Accurately SNOMED-coding data on EMIS Web into the patient record
  • Identifying relevant clinical information documented in patient correspondence, coding appropriately and completing any detailed actions.
  • To be responsible for scanning, coding and summarising the incoming clinical and non-clinical information and is correctly linked to or embedded in the patient records.
  • Input and extract data as required to support practice reports
  • Input data into the patient’s healthcare records as necessary
  • Support all clinical staff with general administrative tasks as requested
  • Work closely with the Frailty, Practice teams and other care coordinators to assist with Nursing and Residential ward rounds.
  • Support patients to utilise decision aids and tools in preparation for a shared decision-making conversation.
  • Help people to manage their needs through answering queries, signposting to available services, and ensuring that people have good quality written or verbal information to help them make choices about their care.
  • Provide coordination and navigation for people and their carers across health and care services, working closely with social prescribing link workers, health and wellbeing coaches, and other primary care professionals across the PCN.
  • Explore and assist people to access personal health budgets where appropriate including providing information on both local and national projects for example sustainability and improvement projects in place.
  • Holistically bring together all of a person’s identified care and support needs, and explore options to meet these using all available services and resources.

  • Be able to communicate effectively in person, over the phone and via electronic means
  • Ability to prioritise and finish work tasks in timely manner
  • Be proficient with computers to complete administrative functions

Job summary

Dene & Stour Valleys PCN seek an enthusiastic and proactive Paramedic tjoin their team. Paramedics work autonomously within the community at an advanced level of practice, using their enhanced clinical assessment and treatment skills, tprovide first point of contact for patients presenting with undifferentiated, undiagnosed problems relating tminor illness or injury, abdominal pains, chest pains and headaches. They are health professionals whpractice at an advanced level having the capability tmake sound judgements in the absence of full information and tmanage varying degrees of risk when there are complex, competing or ambiguous information or uncertainty.

Main duties of the jobs

  • assess and triage patients
  • advise patients on general healthcare and promote self-management where appropriate, including signposting patients tother community or voluntary services
  • be able to:
  • perform specialist health checks and reviews
  • perform and interpret ECGs; alongside other results as appropriate
  • perform investigatory procedures as required
  • Undertake the collection of pathological specimens including intravenous blood samples, swabs, etc.

Job summary

Dene & Stour Valleys PCN are looking to recruit Clinical Pharmacist who is passionate for delivering a quality service to their patients. The Clinical Pharmacist role supports to improve patients’ health outcomes and of the efficiency of the primary care team by providing direct, accessible and timely medicines expertise, including face to face. Clinical Pharmacist proactively transfer workload relating to medicines optimisation issues to improve patients care safety and free up clinical staff to spend more time on clinical care.

Main duties of the job

The post holder will help patients to get the best from their medicines by switching medications to agreed and approved protocols, improving repeat prescribing processes in General Practice, including promotion of repeat dispensing and online ordering, minimising clinical risk and aiming to reduce wasted medicines.

  • To work as part of a multi-disciplinary team in a patient-facing role to clinically assess and treat patients using extensive experience and in-depth knowledge to be directly responsible for care and prescribing of defined cohort(s) of patients
  • To deliver and develop pharmacy led medicines optimisation working with general practice and the wider multidisciplinary team, ensuring practice remains up to date
  • To lead and promote, support and facilitate medicines optimisation for specialist area in the wider medicine’s agenda ensuring best value, reduced harm and improved outcomes.
  • To lead and deliver evaluation and audit of service delivery for specialist area in order to ensure a high-quality service and to further develop provision.
  • To work to the principles of Medicines Optimisation and relevant up to date national guidelines and evidence base.
