GP Letters / Medical Reports

If you'd like to request a GP letter or a medical report you can send us your request online here. There are usually charges for these letters or reports, because this is not included in NHS care and your doctor will need to spend additional time providing this service.

If you need to arrange an appointment for a private medical examination (e.g. for renewal of an HGV licence), please send us your request online here and we will contact you back to make an appointment if needed.
